Hospital discharge equipment
Coming home from the hospital? Have the equipment ready.
When a discharge date is set, the equipment has to be arranged too. Families and care managers call us to have the bed, wheelchair, lift or oxygen in place at home — delivered, set up, and ready.
The discharge lineup
Start with the need, not the catalog.
Discharge plans usually call for equipment in three groups. We rent all of it by the month:
Hospital beds, trapeze bars and overbed tables — the setup most recoveries are built around.
Wheelchairs, patient lifts and power chairs for getting around the home safely.
Oxygen concentrators, portable oxygen and tanks. Oxygen always requires a prescription — ask the care team to include it in the discharge paperwork.
Two things worth knowing up front: walkers and rollators are sold, not rented — if the care team recommends one, ask us about purchasing. And Central Home Care supplies equipment only: we do not provide home health aides, nursing or other in-home care services.
From discharge plan to home setup.
One call starts the delivery. Our Lynbrook team coordinates the details.
- Tell us the discharge date. Share what the care team recommended and when the equipment needs to be in place.
- We check the details. We confirm current inventory and handle the prescription and insurance verification.
- We deliver and set up. Your equipment is delivered, assembled, and checked before we leave.

Questions, answered.
For availability or a situation specific to you, call our Lynbrook team directly.
What equipment do people usually need after a hospital discharge?
Most discharge setups combine a hospital bed with positioning support (trapeze bar, overbed table), a wheelchair or patient lift for moving around, and oxygen when the care team orders it. Tell us what the discharge plan recommends and we will confirm availability.
Can I arrange the equipment before the discharge date?
Yes. Call us with the discharge date and what the care team recommended, and we will coordinate delivery so the equipment is in place and set up at home.
Who can set up a discharge rental — family or a care manager?
Either one. Family members, discharge planners and care managers all arrange rentals with us by phone. If you are coordinating for someone else, one call is all it takes to start.
Does insurance cover hospital discharge equipment?
Many items can be insurance-billed with a valid prescription — we are a Medicare-enrolled supplier and verify coverage before delivery. Monthly self-pay rental is the other route. See prescription requirements for which route needs what.
Do you provide a nurse or home health aide to help at home?
No. Central Home Care is an equipment supplier — we deliver, set up and service the equipment. Hands-on care is provided by family members or a home health agency.
